Section 4: Equal Opportunities Monitoring Form

We aim to provide fair and equal access to appointments and to ensure that all sectors of society are represented at EW Group.

You are asked to complete an Equal Opportunities Monitoring Form. The questions set out in the form below help us to monitor the effectiveness of this policy: by gaining a picture of all those applying for and obtaining appointments. They also help us to monitor how we are complying with UK equality law.

The Equality Act 2010 protects people from discrimination and promotes equality on the basis of a number of ‘protected characteristics’. We ask for information on your protected characteristics to help us monitor our performance on workplace equality, diversity and inclusion.

In accordance with the provisions of the Data Protection Act 1998, the information you provide will be held confidentially and can only be used if you give us your consent. We are aware of the discrimination that exists in relation to socio-economic factors. We ask that you respond to the questions below, even though this form of discrimination is not covered by law.

The form will be kept separate from your application and will not be seen by the shortlisting or interview panel.

Assessment of your suitability for the post is made purely on the information you give on the application form, your performance at the interview and in any related tests, should you be invited. Appointments are made strictly on merit.

At birth, which of the following were you described as? *

Which of the following describes how you think of yourself? *

The Equality Act 2010 defines a person as disabled if they have a physical or mental impairment which has substantial and long term (i.e. has lasted or is expected to last at least 12 months) adverse effect on their ability to carry out normal day-to-day activities. Adverse effects may arise from external barriers experienced by people with impairments.

When you answer the question, you should not take into account the effect of any medication or treatments used or adjustments made (for example, at work or at home) that reduce the effects of impairments. Instead, you should think about the effect the impairment would have if these were not being used or made.
Do you have a disability (as defined by the Equality Act 2010)? *

EW Group guarantees an interview to all candidates with a disability who meet our minimum criteria (70%) for the job vacancy.

If you have a disability (as defined by the Equality Act 2010), do you wish to apply under the guaranteed scheme? *

Please note that the guaranteed scheme is available to disabled candidates only. Regrettably, any false declaration of disability in order to secure an interview will impact on your overall application.

If you have a disability or health condition, we wish to ensure that we make reasonable adjustments to the recruitment process.

Do you wish to request adjustments to the recruitment process? *

Do you require any other special arrangements (not covered above), throughout the recruitment process? *

Please note that, prior to participation in subsequent stages of the recruitment process, we will ask you to re-confirm if there are any special arrangements you require, including reasonable adjustments.
